Your employer needs to send us from his payroll system a Form 100 – an abstract of your pay slips. Please make sure that your employer did so.
If the employer failed to send a Form 100 and there is no way to send the form through him, you must attach an employer certificate regarding the period of employment and pay (BL/1514)
or
Letter on work termination of from the employer – indicating the reason of work stoppage (such as dismissal, resignation, departure on unpaid leave, retirement, etc.) and the date of the termination, with the signature and stamp of the employer, and pay slips for 12 months of work in the last 18 months preceding registration with Employment Service – including the last 6 months of work. Attaching the pay slip of the last month of work is mandatory.
